Pulse therapy was done since Oct. 20 to (until) Oct. 25.

since----to (until)---- 不是英文的表達方式，但屢見於病歷中。建議改為：
Pulse therapy was done from Oct. 20 to (until) Oct. 25.
Pulse therapy was done between Oct. 20 and Oct. 25.
寫得花俏一點：
Pulse therapy initiated on Oct. 20 and had been given until Oct.25 (後半句用過去完成式以強調從 "較遠的過去" 到 "較近之過去").

如果寫成 Pulse therapy was done since Oct. 20. 也不對
since (作"自從"解) 之前應該接 "現在完成式" 或接以 "It is ----" 所領導之句子 (e.g. It is three years since we met in New York)，用來表達一件事從過去延續至今。

若寫成 Pulse therapy has been done since Oct 20. 用以表達從 Oct.20 至今，文法正確--但語意是Pulse therapy已結束---"has been done".

Pulse therapy has been given since Oct. 20. 這樣換個動詞來 "說明時間" 就表示 Pulse therapy 自 Oct.20 迄今仍在進行.

since and from (從-----起)
病歷中屢見如下之錯誤寫法:
Chemotherapy started since Oct. 25.
Vancomycin was given since Oct. 25.

正確之寫法為
Chemotherapy started from Oct. 25
Vancomycin was given from Oct. 25 to Oct. 28.
Vancomycin has been given since Oct 25. 意為Vancomycin 自 Oct. 25 起使用至今.

又 since 後所接的是 "過去之時間"
Dexan will be given since Oct. 30. 則大錯特錯了.
正確的寫法為 Dexan will be given from Oct. 30.

Since two days ago (自從兩天以前)，這不是英文的習慣用法，像是從中文衍生的寫法。
明天是 228，你有要事必須前往郵局，因此拿起電話問郵局 "你們明天開門嗎?"----
Q: Do you open the door tomorrow?
A: What door do we need to open?　

所以建議 “你們明天開門嗎?” Q: Is your office open tomorrow?

Since two days (five years) ago 在病歷中隨處可見.
例如："She suffered from rheumatic heart disease since five years ago". 作者本意為 She had rheumatic fever five years ago and has suffered from rheumatic heart disease since then.

since 之前宜用完成式；ago 之前宜用過去式。那要如何表達 "He suffered from abdominal pain since two days ago" ? 以下是我常用的寫法：
He has suffered from abdominal pain for two days.
He has suffered from abdominal pain since two days prior to (before) this admission.
